At Zone 5 Physical Therapy, care isn’t dictated by insurance companies. We’re a self-pay, cash-based physical therapy clinic focused on one-on-one treatment, individualized plans, and real results.
**Payment is due at the time of your visit. Cash is appreciated, and Apple Pay and Venmo are accepted. You’ll receive a detailed receipt with NPI, diagnosis, and CPT codes, which may be submitted for HSA reimbursement.
Utah allows direct access to physical therapy, meaning you can book an appointment and begin treatment without a physician referral. If we identify anything outside the scope of physical therapy, we’ll help guide you to the appropriate provider.
